Terms and Conditions


This web site (“Site”) and related services are provided by CommUnityCare to its patients subject to compliance with the terms and conditions set forth below.  Please read the following information carefully.  Your continued use of this Site will indicate your agreement to be bound by the terms and conditions set forth below.  If you do not agree to be bound by these terms and conditions, promptly exit this Site.  

 

Privacy and Security Responsibilities

When you sign up to use MyChart, you will receive an activation code.  You will use that code to create an ID and password.  This ID and password will allow you to view select portions of your health record, submit comments that may become part of your health record, and in some cases communicate with your health care providers.

 

It is important that you keep your access code, ID and password completely confidential and secure.   You should change your password immediately if you believe that your security may have been compromised.  In addition, you should change your password regularly to help keep your account secure.  You can change your password online at any time.  The privacy of your health information is carefully protected by your health care providers.  

 

CommUnityCare will, at all times, maintain your confidential medical information in strict confidence and will not disclose any information regarding you to any unaffiliated third party unless you authorize that person to receive your information or the information is permitted to be disclosed by law.  Please review CommUnityCare’s Notice of Privacy Practices for a thorough description of how we gather, use and protect your confidential medical information.  All messages sent and received within MyChart that contain medical information are subject to all state and federal laws governing the security and confidentiality of medical records.

 

MyChart Features

 Electronic Messaging

CommUnityCare offers MyChart to its patients as a convenience and not as a substitute for face-to-face consultation with your provider. 

 

By using MyChart, I understand and agree to the following: 

  • I will not use MyChart until I have established a relationship with a CommUnityCare provider. 
  • I acknowledge MyChart is not a substitute for personal consultation with my provider. 
  • I will make an appointment with my provider should I have any changes in my health or the condition for which I am being treated for by my provider. 
  • I understand and agree MyChart is NOT to be used to communicate urgent or emergency health issues. In the case of an emergency, I will go to the nearest Emergency Department or call 911. 
  • I agree to not change or stop any course of treatment prescribed by my provider based on information found on MyChart without consulting my provider first. 
  • I understand that messages sent via MyChart might not be addressed immediately and may be routed to another authorized caregiver within CommUnityCare in order to facilitate a timely response. 
  • I understand that my provider may not review and respond to a message that I send via MyChart on the same day, after normal working hours, or on holidays or weekends. I will not make any healthcare decisions without consulting my provider first.
  • I understand that computers may malfunction and that MyChart may not be available 24 hours a day, 7 days a week without interruptions. I understand that if I am unable to access MyChart, I can contact my care team by telephone.  
  • Discussion of sensitive healthcare matters such as mental health or substance abuse, HIV or STDs should occur in face-to-face conversations with my provider and not through MyChart. 
  • I understand that my activities within MyChart may be tracked by computer audit and that entries I make may become part of my medical record. 
  • I understand that CommUnityCare has the right to deactivate my access to MyChart at any time for any reason. 
  • I understand that use of MyChart is voluntary and I am not required to use MyChart or to authorize a MyChart proxy. 

 

Secure Communications

All communications between you and CommUnityCare using MyChart occur over a secure connection. You will receive Internet e-mail messages notifying you that new information is available in your MyChart account.  These e-mail messages, however, will not contain any confidential medical information. All of the confidential medical information available to you in MyChart is protected and securely maintained by CommUnityCare.

 

Credit Transactions 

You may elect to pay for any service in MyChart with your credit card. By requesting the credit card payment option, you hereby authorize CommUnityCare to charge the credit card for any fees and/or any other amounts you authorize. In the event that your credit card expires, you will be notified the next time you attempt to use that card for payment.

 

Restrictions for MyChart Records of Minors

Parents and legal guardians may request access to their minor child’s health information through MyChart.  This is a type of proxy access (access by one person to another person’s interactive health record).  The amount of information the parent will be able to see depends on the age and legal rights of the minor.  The limits on interactive health record access does not affect any other right the parent may have to obtain a minor child’s records.

 

Termination of Access

CommUnityCare may restrict or terminate access to MyChart at any time.  If access is restricted for planned maintenance of the MyChart system, it will be communicated via a notification on MyChart Homepage. 

 

Misuse of the MyChart application or violation of these Terms and Conditions may, in the sole discretion of CommUnityCare, result in CommUnityCare suspending or terminating your access privileges. Improper activities include, but are not limited to, the use of inappropriate or abusive language, harassing behavior, unauthorized attempts to access patient records, frequent cancellation of appointments reserved through MyChart, and other activities that cause or are intended to cause a disruption to medical operations, MyChart operations, or CommUnityCare’s technology or medical records systems, as determined in the sole discretion of CommUnityCare.

 

Use and Ownership

All pages within this Site and any material made available for download are the property of CommUnityCare.  The content on the Site is provided for CommUnityCare and its patients only.  The words “CommUnityCare” and its associated corporate logos are the trademarks and service marks of CommUnityCare.  All other trademarks, service marks, and logos used in this Site, including but not limited to MyChart and Epic, are the trademarks, service marks, or logos of their respective owners.  Nothing on this Site should be construed as granting any license or right to use any trademark, service mark, or logo displayed on this Site without the express permission of CommUnityCare or the third party that may own the trademark, service mark, or logo.  

 

Accuracy and Integrity of Information

Although CommUnityCare attempts to ensure the integrity of the Site, it makes no guarantees whatsoever as to the correctness or accuracy of the Site. It is possible that the Site could include typographical errors, inaccuracies or other errors. Please contact your provider’s office if an inaccuracy arises. Information contained on the Site may be changed or updated without notice.
